Introduction and Scope

Health and Wellness Technologies (HWT) is a consultant for health and wellness providers and acts as a "Business Associate" under HIPAA through signed Business Associate Agreements (BAAs). We safeguard Protected Health Information (PHI) according to federal, UCPA and HIPAA standards. When handling PHI for clients, it may be for "healthcare operations", as specified in a BAA or through a signed Patient Informed Consent form by Provider and Patient. PHI is never sold or used for marketing without written authorization.

Information Collected

HWT may collect personal data on clinic owners, providers and staff like name, email, phone and address; business data like clinic details, financial records, patient volume and operational workflows; and sensitive data only through a signed, limited, opt-in Informed Consent Form mutually approved by provider, patient and business associate.

For information requested or submitted online by “Contact” form, email or other electronic means, we may save the personal and other data you may provide. We may use this information to contact you about solutions or services we feel may benefit you, including contact preference, opt-out links. HWT respects the privacy of those who visit our website. We do not now nor intend in the future share, sell, or rent personally identifiable information to any third party.

Information Security

All data is securely stored and accessed through first tier software that uses Multi-Factor Authentication (MFA) and provides end-to-end encryption for all data transmissions. While our office computers do usually contain any personal or sensitive data, they are all encrypted and password protected behind locked doors.

Information Collected by Others

This notice applies exclusively to the HWT site and not to any sites that users may access via links from our site. HWT is not responsible for the information collecting policies, practices employed or the information or content of other sites. Links to other websites may contain useful information for our visitors, however Users are advised to review the privacy policy of other websites.

We may use cookies to help compile aggregate site traffic and interaction data to provide a better User experience. We may also use trusted third-party services that track this information on our behalf.
